How passqual.com collects, uses, and protects the information you share with us online.

This Privacy Policy explains how PassQual Health ("PassQual Health," "we," "us," or "our") collects, uses, and protects information through this website, passqual.com (the "Site"). It applies to the Site only — not to the medical records or protected health information we create and maintain when providing you with clinical care. For that, please see our Notice of Privacy Practices.

By using this Site, you agree to the practices described in this Policy. If you do not agree, please do not use the Site, and contact our front desk directly using the information below instead.

We collect information in two ways: information you give us directly, and information collected automatically as you browse the Site.

  • Contact form. When you use the contact form on our Contact page, we collect your name, email address, phone number, and the message you write. This form is intentionally limited to non-clinical questions — it asks you not to include medical details, and directs urgent or clinical questions to a phone call instead.
  • Automatically collected technical information. Like most websites, we automatically receive some technical information from your browser or device when you visit — such as your IP address, browser type, device type, the pages you view, and how you arrived at the Site — through the analytics tools described below.

We use the information described above only to:

  • Respond to the message or question you send us through the contact form;
  • Operate, maintain, and improve the Site, including understanding which pages are useful to patients; and
  • Protect the Site and our practice from misuse, fraud, and security threats.

We use Google Tag Manager to load analytics tools that help us understand how visitors use this Site — for example, which pages are viewed most and how visitors navigate the Site. These tools may use cookies or similar technologies stored in your browser.

We do not use this information to identify you personally, and we do not combine it with the contact information you may separately give us through the contact form. You can control or delete cookies through your own browser settings; doing so may affect how some parts of the Site function.

The scheduling tool on our Schedule an Appointment page is a widget provided by a third party, CharmHealth (operated by MedicalMine, Inc.), embedded directly in our pages. When you use it to request or manage an appointment, the information you enter is collected and handled by that provider under its own privacy policy and terms — not this one. We encourage you to review that provider's own privacy policy before using the scheduling widget.

If you would rather not use the online scheduling widget, you can always call us directly to schedule an appointment.

The accessibility menu on this Site (text size, contrast, motion, and related display preferences) stores your choices only in your own browser's local storage, on your own device. These preferences are never transmitted to us, to any analytics tool, or to any other third party — they simply make the Site display the way you prefer the next time you visit, on that same device and browser.

We use industry-standard safeguards to help protect information submitted through this Site, including encrypting the connection between your browser and our servers (HTTPS). No method of transmission over the internet is completely secure, and we cannot guarantee absolute security.

We do not sell, rent, or trade the personal information collected through this Site to any third party for their own marketing purposes.

You may ask us to delete the information you submitted through the contact form, or ask what information we hold about you from that form, at any time by contacting us using the information below. We will respond to your request within a reasonable time.

This Site is intended for use by adults seeking information about our practice and is not directed to children. We do not knowingly collect personal information through this Site from children under 13.

We may update this Privacy Policy from time to time. The effective date at the top of this page reflects the most recent revision. We encourage you to review this page periodically.
