What is Behavioral Health Integration?

Behavioral Health Integration means your mental and emotional health is cared for as part of primary care, by the same team that already knows you — instead of being referred elsewhere and waiting weeks, you begin care where you already are.

It follows the Primary Care Behavioral Health (PCBH) approach: behavioral health is woven into everyday medical visits, which lowers stigma, removes barriers, and helps more people actually get the help they need.

How the Collaborative Care Model works

The Collaborative Care Model (CoCM) is one of the most researched approaches in medicine, shown across dozens of clinical trials to improve depression and anxiety outcomes. Your care is delivered by a team: your primary care provider leads your overall treatment plan; a behavioral health care manager checks in with you regularly, supports your goals, and coordinates your care between visits; and a consulting psychiatrist reviews your progress with the team and advises on the most effective treatment, without you needing a separate specialty appointment.

We practice measurement-based care: brief, validated check-ins like the PHQ-9 for depression and GAD-7 for anxiety track how you're really doing, so we can adjust your plan until you feel better — not just once, but over time.
