USCIS Civil Surgeon & Immigration Medical Exam (Form I-693) in Miami Gardens

Form I-693, the Report of Immigration Medical Examination and Vaccination Record, is how USCIS confirms that a green card applicant isn't inadmissible on health-related grounds. Only a physician USCIS has designated as a civil surgeon — not your regular doctor — may perform the exam and sign the form.

For most healthy applicants it's a straightforward one-visit process. The exam screens for a specific, defined set of conditions under immigration law — certain communicable diseases, missing required vaccinations, and a small set of other health grounds — not a general pass-or-fail physical.

Most people applying for lawful permanent residence from inside the United States need an I-693 signed by a civil surgeon — that includes applicants filing Form I-485 through family, employment, or asylum; refugees and asylees applying for their green card after one year; and anyone USCIS specifically instructs to submit one. (Applicants still outside the U.S. see a panel physician abroad instead — a separate process.)

USCIS only accepts a Form I-693 performed and signed by a designated civil surgeon; an exam from a walk-in clinic or family doctor without that designation is rejected, no matter how thorough it was. The visit covers a review of your medical and vaccination history, a physical examination (eyes, ears, nose, throat, heart, lungs, abdomen, lymph nodes, and skin, plus a brief mental-health review, as CDC instructions require), a tuberculosis blood test for applicants age two and older, and the blood testing the CDC requires for certain adult age groups. When everything is complete, you receive your signed Form I-693 in a sealed envelope exactly as USCIS requires, plus a personal copy for your own records. Do not open the sealed envelope — USCIS will not accept one that has been opened or altered.

USCIS requires proof of age-appropriate vaccination based on CDC guidelines — depending on your age and history, that can include MMR, Td/Tdap, polio, hepatitis A and B, varicella, and seasonal influenza, among others. As part of your exam, we review your vaccination record against those requirements and tell you exactly which vaccines, if any, you are still missing.

If a vaccine is missing, we point you to where you can get it at no cost — typically the county health department — or, when appropriate, a blood titer test can document immunity you already have.
