If you live in Miami Gardens or Hialeah and you have no Medicare, Medicaid, commercial or marketplace coverage, you have probably put off seeing a doctor. Not because you don’t care, but because you can’t predict the bill. Many people search for “doctor without insurance near me” and find confusing prices or no clear answer at all. This article explains, plainly, how the PassQual Membership works: what it includes, what it costs, what it does not cover, and who can join.

First, the most important part: it is not insurance

The PassQual Membership is not health insurance. It is a direct health care agreement under §624.27 of the Florida Statutes. You pay a fixed monthly fee directly to the clinic, and in return you get a clear set of primary care services. It does not replace insurance, it does not match insurance, and it is not a substitute for it. It is an option for people who have no active coverage today and need a trusted doctor while they decide their next steps.

Who can join?

The membership is only for self-pay patients, meaning people without coverage of any active kind. If you have Medicare, Medicaid, a marketplace plan, an employer plan or commercial insurance, you cannot enroll. This rule protects everyone: it keeps anyone from paying twice for the same care, and it makes clear from day one who the membership is for.

Why this matters more in 2026

With the end of the enhanced ACA subsidies starting in 2026, KFF (kff.org) estimates that premium payments for subsidized marketplace enrollees could more than double on average nationwide: a 114% increase, from $888 to $1,904 a year. In Florida, reporting from Florida Phoenix and Orlando Weekly describes double-digit premium increases for 2026. These figures are averages and do not apply the same way to every person. But they help explain why more families in our community are losing coverage. If you can no longer afford your plan, you still have options: you can have a fixed, transparent monthly price for your primary care.

The three levels and their prices

Prices are fixed and public. There are no surprises.

  • Core Care: $79 a month, or $790 a year prepaid.
  • Plus Care: $119 a month, or $1,190 a year prepaid.
  • Elite Care: $159 a month, or $1,590 a year prepaid.

You can choose a 6-month plan billed monthly, or an annual prepaid plan that gives you about two months free. If you need to spread out the cost, financing is available through CareCredit.

What every level includes

  • Primary and preventive care visits: in the office, by telehealth and by messaging.
  • A comprehensive annual physical exam.
  • Same-day or next-day priority scheduling, with a care team that speaks Spanish and English.
  • Routine labs.
  • One X-ray when clinically needed.

In practice, this means you can message your care team when you have a question, check your blood pressure or blood sugar regularly, and talk through your results calmly, without watching the clock or the bill.

What it does not cover

We would rather say it clearly now than have you find out at the worst moment. The membership does not cover emergencies, hospital stays, specialists, prescription drug coverage or third-party services. If you have an emergency, call 911 or go to the nearest emergency room. If you need a specialist, your doctor will help you understand your options, but that cost is separate.

We also do not promise outcomes. The agreement states clearly that no clinical result is guaranteed. What we do offer is real access to your doctor, time to listen to you, and a care plan explained in simple words.

How to cancel

You can cancel at any time with 30 days of written notice. If you cancel before the end of your term, a one-time $99 fee applies. There is no hidden fine print: these are the full rules.

Who will care for you

The membership is provided by Dr. Hendry Pascual, Florida license ME143678, at our clinic in Miami Gardens. Dr. Pascual is a USCIS-designated civil surgeon, completed his residency at UCLA, and holds the CHCQM-PHYADV certification. His bilingual team explains every step without medical jargon, so you understand your health and can make decisions with confidence.

Is it right for you?

The membership may make sense if you have no active coverage, want a regular doctor, prefer to know the price before you arrive, and value being able to send a message or have a telehealth visit without paying each time. If you still have insurance, or are about to enroll in a plan, it is best to use it; the membership is not for you in that case. If you are not sure which level fits you, call us and we will walk you through it with no pressure.
