If you already pay for health insurance, the idea of paying a monthly fee to a primary care practice can sound like paying twice. It’s the most common objection we hear about direct primary care in Miami, and it’s a fair one. The honest answer depends on what you actually need from your doctor — and on understanding one thing clearly before anything else.

First, the part that matters most: membership is not insurance

A direct primary care membership is not health insurance, and it does not replace it. It doesn’t cover hospital stays, emergency room visits, surgery, specialist care, or the large, unexpected bills that insurance exists to protect you from. Florida law treats a direct primary care agreement as a separate arrangement between you and a practice, not as an insurance product.

So the real question isn’t “membership or insurance.” For most people it’s “insurance, and is a membership worth adding on top?”

What insurance is built to do — and what it isn’t

Insurance is designed around risk: the rare, expensive event. It’s very good at that. What it was never designed around is access — how quickly you can see your own doctor, how much time you get in the room, and whether you can ask a simple question without booking a full visit.

That gap is where many people feel the friction of everyday primary care:

  • Waiting weeks for a routine appointment, then getting a short visit once you’re there.
  • Ending up at urgent care for something your own doctor could have handled, because no one could fit you in.
  • Putting off a question about a new symptom or a medication because it doesn’t feel “worth” a visit.
  • Seeing whoever is available instead of a care team that knows your history.

Direct primary care is built to address that access gap. It is not built to address catastrophic risk — that’s still insurance’s job.

When a membership tends to make sense alongside insurance

A membership is usually worth a closer look if one or more of these sound like you:

  • You see your primary care team often. If you’re managing an ongoing condition like high blood pressure or diabetes, regular contact and easy follow-up matter more than they do for someone who visits once a year.
  • You value speed and continuity. Priority scheduling and a consistent, bilingual care team can mean fewer gaps in care and less time explaining your history from scratch.
  • You want wellness and access benefits your plan doesn’t include. Memberships are designed to cover the access and convenience that insurance typically leaves out.
  • You’re self-employed or between plans. Some people use a membership for everyday primary care while keeping separate coverage for major events. Even then, you still want coverage for hospital and emergency care.

When it may not be worth it for you

To be equally direct: if you’re healthy, rarely see a doctor, and are happy with how quickly your current practice gets you in, a membership may not add much for you right now. It’s also important not to drop insurance because you’ve joined a membership — the two do different jobs, and going without coverage leaves you exposed to exactly the costs a membership doesn’t touch.

Questions to ask any direct primary care practice

Whether you choose us or another practice, ask these before you sign up:

  • Exactly what’s included each month, and what’s billed separately?
  • How does the membership work alongside my insurance for services my plan covers?
  • How fast can I realistically get seen, and how do I reach my care team between visits?
  • Is care available in my preferred language?
  • What agreement will I sign, and does it clearly state that this is not insurance?

A good practice will answer all of these plainly and in writing. If anyone suggests a membership can replace your health insurance, treat that as a red flag.

How PassQual approaches it

At our Miami Gardens practice, the PassQual Membership is designed around access: priority scheduling, more ways to reach your care team, and bilingual English/Spanish care. It is a medical membership, not insurance, and it does not replace your health plan. If you have coverage, you can review the insurance plans we accept to see how your existing plan fits in.
