If you’re a CDL driver with sleep apnea, diabetes, or a vision or hearing issue, it’s normal to feel nervous before a DOT physical. The good news: none of these conditions automatically disqualifies you from certification, and your exam at PassQual Health is still a flat $50 — no extra fee for a more complex case.
This guide covers what the Federal Motor Carrier Safety Administration (FMCSA) generally expects you to disclose, what documentation might help, and what to bring with you. For your specific situation, the fastest and most accurate answer comes from talking directly with our examiner — not from a website.
Do I Have to Disclose Sleep Apnea, Diabetes, or Vision/Hearing Problems?
Yes. As part of every DOT physical, FMCSA requires commercial drivers to answer health-history questions honestly and completely. That includes current conditions like sleep apnea, diabetes (insulin-dependent or not), and vision or hearing issues, along with any medications you take.
Leaving something off the form isn’t a shortcut — it can create bigger problems later. The safer approach is to disclose the condition and let the exam process work the way it’s designed to.
Does Having One of These Conditions Automatically Disqualify Me?
No. Sleep apnea, diabetes, and vision or hearing issues are all conditions FMCSA asks about — but none of them is an automatic disqualifier on its own. Certification is based on your individual health history, how well-managed your condition is, and sometimes additional documentation.
What matters most is coming prepared and being straightforward with the examiner. Guessing at what you think the answer “should” be, or leaving out details, only makes the process harder.
What About Sleep Apnea?
If you’ve been diagnosed with sleep apnea, or your examiner flags risk factors for it during your exam, that alone doesn’t disqualify you. The examiner will ask about your diagnosis and treatment, and may want to see documentation from your treating physician — for example, confirmation that you’re under care and following your treatment plan.
If you use a CPAP machine or another treatment, bring any records or compliance reports you have. If you’re not sure what documentation is enough for your case, ask our examiner before your exam — don’t guess.
What About Diabetes — Insulin-Dependent or Not?
Both insulin-dependent and non-insulin-dependent diabetes are conditions FMCSA asks about at every DOT physical. Neither one automatically disqualifies you. Depending on your treatment history, the certification process may involve extra questions or a request for records from your treating physician.
If you manage diabetes with insulin, additional steps may apply as part of your certification. Bring your current treatment plan, recent lab results if you have them, and any letters from your physician — and ask our examiner exactly what’s needed for your situation.
What About Vision or Hearing Issues?
FMCSA sets vision and hearing standards that are checked as part of every DOT physical. If you wear glasses, contacts, or a hearing aid, wear them to your exam — vision and hearing are typically tested with your corrective devices in use.
If your results raise a question on exam day, our examiner will walk you through what options may apply. We can’t tell you in advance whether you’ll meet a specific standard — that determination happens at the exam, based on your actual results that day.
What Should I Bring to My Exam?
- A valid photo ID
- A list of current medications and dosages
- Glasses, contacts, or hearing aids, if you use them
- Any letters, notes, or records from your treating physician about a diagnosed condition
- Treatment records for sleep apnea (such as CPAP compliance data), if applicable
- Recent lab results or a treatment summary, if you manage diabetes
Bringing this paperwork doesn’t guarantee a specific outcome, but it gives our examiner what’s needed to evaluate your situation accurately and efficiently.
Does a Medical Condition Change the Price of My DOT Physical?
No. Your DOT physical at PassQual Health is a flat $50, whether or not you have sleep apnea, diabetes, or a vision or hearing issue. There’s no separate facility fee or paperwork fee. Your exam is performed by an FMCSA-certified medical examiner listed on the FMCSA National Registry, with electronic transmission of your results to Tallahassee and a laminated pocket DOT medical card included.
For general pricing details, see our DOT physical cost page.
Managing These Conditions Beyond Exam Day
Sleep apnea and diabetes are conditions best managed with ongoing primary care, not just at your annual DOT physical. If you need a primary care provider in Miami Gardens who can help you stay on top of your treatment year-round, our bilingual primary care team can help.
Frequently Asked Questions
Will sleep apnea automatically disqualify me from a DOT physical?
No. A sleep apnea diagnosis or risk factors alone don’t automatically disqualify you. The examiner reviews your individual history and treatment, and may request documentation from your treating physician.
Can I still get DOT certified if I take insulin for diabetes?
Insulin-dependent diabetes doesn’t automatically disqualify you from certification. It may involve extra steps as part of the certification process. Bring your current treatment records and ask our examiner what applies to your case.
What if I wear glasses, contacts, or a hearing aid?
Bring and wear them to your exam. Vision and hearing are generally tested with corrective devices in use. Ask our examiner directly if you have questions about your specific results.
Do I have to tell FMCSA about these conditions?
Yes. FMCSA requires drivers to answer health-history questions honestly and completely at every DOT physical, including conditions like sleep apnea, diabetes, and vision or hearing issues.
Does having a medical condition change the cost of my exam at PassQual?
No. The exam is a flat $50 regardless of your health history, with no separate facility or paperwork fee.
