    Your civil surgeon can document a blanket waiver directly on Form I-693, without a separate application, when:

      Religious or moral conviction: a separate process — the applicant must object to all vaccinations, not just specific ones, and files USCIS Form I-601 or I-602 alongside the case. Your civil surgeon can explain whether this applies to you.
